Ready to Drive Life-Changing Transformations Through Evidence-Based Coaching?

We're seeking a passionate health professional who can serve as a trusted primary coach within Personify Health's Transform Program. As our Part-Time Transform Health Coach, you'll support participants through weight management, blood pressure management, diabetes prevention, and our GLP-1 Companion Program using multimodal coaching interactions and evidence-based frameworks.

What You'll Actually Do

Provide specialized coaching: Deliver virtual health coaching for Transform Program participants supporting weight management, blood pressure management, diabetes prevention and management, including GLP-1 Companion Program.

Guide structured programs: Connect with participants through multimodal coaching interactions to guide them through year-long program and curriculum using evidence-based frameworks.

Apply whole-person strategies: Use motivational interviewing and appreciative inquiry to support participants' holistic wellbeing and drive sustainable habit change.

Monitor participant success: Track progress, health outcomes, and milestone achievement using data and trends to inform coaching strategies and contribute to program effectiveness.

Maintain comprehensive documentation: Document coaching interactions accurately using dynamic coach dashboard to track progress, schedule sessions, and maintain participant records for outcomes reporting.

Drive performance excellence: Meet individual and team performance goals while contributing to culture of learning, quality, and continuous improvement.

Support chronic disease management: Help participants manage health and prevent chronic disease by addressing wide range of concerns, fostering self-efficacy, and connecting them to appropriate resources.

Engage in professional development: Complete required training and continuing education to maintain program fidelity and stay current with NBC-HWC and DPRP standards.

 

Schedule & Training Requirements

Intensive Training Period (First 8 Weeks):

  • Weeks 1-2: Full-time training (40 hours/week, 8am-5pm CT)
    • Dates: December 8th 2025 - January 30, 2026
    • No time off permitted during initial training period
  • Weeks 3-8: Continued training + coaching work (up to 8 hours training on Wednesdays and Thursdays)

Ongoing Schedule:

  • Hours: Hours can range 16-40 hours based on business needs, on average employees can expect 16-20 hours per week.
  • Required availability: Monday-Thursday 7am-10pm CT, Friday 7am-6pm CT
  • Required meetings: Wednesdays 1-2pm CT, Thursdays 2-3pm CT
  • Coaching format: 20-minute or 40-minute appointments, often back-to-back, plus messaging throughout shifts

Schedule flexibility: We provide at least 1 month's notice for any hour increases or decreases. Team members must accept at least 50% of additional hours offered when business needs increase.

 

Technology Requirements:

  • Internet: Minimum 20 Mbps download / 10 Mbps upload, hardwired connection required.
  • No Wi-Fi, MiFi, or public spaces allowed for coaching.
  • Speed test required during interview: www.speedtest.net.

Qualifications

What You Bring to Our Mission

The educational foundation:

  • Bachelor's degree in health-related field required; Master's degree preferred
  • NBHWC or ICF certification strongly encouraged but not required
  • Preferred credential or licensure in:
    • Behavioral Health
    • Diabetes Management (experience with CGM)
    • Exercise Physiology
    • Registered Dietitian (RD)
    • Registered Nurse (RN) — must be licensed in compact state

The coaching expertise:

  • Minimum 1 year experience providing health coaching through multimodal interactions, with focus on chronic disease prevention and management (diabetes, hypertension, weight management)
  • At least 1 year experience applying behavior change techniques, including motivational interviewing; experience in disease prevention and management is plus
  • 2-3 years experience in fields relevant to health coach role, such as behavioral health, general wellbeing, public health, health promotion, disease prevention and management, or nutrition and physical activity coaching
  • Experience conducting individual or group coaching sessions using primary coach model

The language abilities:

  • Strong proficiency in English, both written and verbal
  • Bilingual in Spanish and English (both written and verbal) is a huge plus

The professional competencies:

  • Digital proficiency: Comfortable using digital health apps, web-based software, and electronic documentation; quick to learn new technologies and systems
  • Coaching expertise: Skilled in holistic, evidence-based coaching approaches that promote participant-led goal setting, self-efficacy, and behavior change
  • Multimodal coaching: Able to effectively support participants through Personify Health's multimodal coaching approach and Transform Curriculum
  • Health knowledge: Preventive care-minded with ability to address wide range of health concerns and drive positive participant outcomes
  • Interpersonal skills: Outstanding communication and rapport-building abilities with empathy, cultural sensitivity, and ability to navigate complex cases

  • Professional commitment to evidence-based practice, continuous development, and program fidelity (including CDC Diabetes Prevention Program standards)

In compliance with all states and cities that require transparency of pay, the base compensation for this position ranges from $27 to $31 per hour. Note that compensation may vary based on location, skills, and experience. This position is part time and therefore not eligible for benefits.
